Review:
Hospitalito Atitlán is a wonderful place, and I would recommend anyone spending time there to anyone who has the opportunity. While I participated in a fellowship program prior to starting medical school, the Hospitalito does accept 4th year medical students with “intermediate level” Spanish for 1 month rotations (https://hospitalitoatitlan.org/medical-students/). Santiago Atitlán has very few English speakers, so it is an excellent opportunity to improve Spanish language skills. I took classes in the local Mayan language, Tzutujil, and the instruction during those classes was excellent! I am planning to do a rotation there during fourth year, and would recommend it to anyone!
